AWARE's Program of Assertive Community Treatment (PACT) psychiatric-mental health nurse practitioner (PMHNP) is responsible for providing leadership and guidance to PACT program staff concerning patient care. This includes providing direct psychiatric care to patients, as well as consultation and guidance to other members of the team. Duties will include, but are not limited to, performing psychiatric evaluations, contributing to team development of patient care plans, obtaining indicated diagnostic tests, prescribing medications and treatments, and providing ongoing psychiatric care. The PMHNP may also carry a small caseload of patients not enrolled in PACT services.
The PMHNP understands that AWARE is not attempting to institute a medical model in its services; rather, the PMHNP works within a team to help people live as independently as possible with dignity and respect, in keeping with the PACT model of care.
