Job Summary:
The Hospice House Manager has general management responsibility for services provided within the Hospice House, with direct supervisory responsibility for the Hospice House clinical staff. This position serves as a pivotal point in communication with clinical staff, support staff, referral sources, and the Associate Director of Hospice House Operations to ensure extraordinary service delivery. This position reviews, plans and evaluates the care of patients in accordance with organizational policies and evidence-based standards of professional clinical practice. This position provides orientation, training and support for both newly hired and existing clinicians. This position serves as a mentor for case management and participates in the assessment and planning of appropriate care.
Essential Duties and Responsibilities:
- Utilizes evidence-based practice to implement standards of care within the Hospice House. Actively works on improvement initiatives with support of the Associate Director of Hospice House Operations.
- Maintains a consistent focus on Key Performance Behaviors (KPBs) to drive favorable clinical and financial outcomes, including providing clear expectations to staff, giving them the tools and education to support best practice, directly supervising and engaging in performance improvement, and creating a culture of accountability within the Hospice House.
- Assists, with the Associate Director of Hospice House Operations, in the hiring of all Hospice House staff. Monitors staffing ratios, ensuring adequate and appropriate staffing for services. Allocates resources appropriately to ensure efficient service delivery 24/7. Ensures monthly posting of staff schedules according to agency protocols and collective bargaining agreement.
- Coaches, mentors, and trains/develops staff, oversees new employee on-boarding and orientation, performance management, evaluation and disciplinary action if needed.
- Facilitates patient and employee scheduling/assignment, including rapid admissions process, in collaboration with the clinical team to ensure timely initiation of care and timely patient transfers into the Hospice House.
- Assists in the performance of patient visits and patient care within the Hospice House when the need arises to ensure appropriate staffing coverage.
- Provides direct supervision of clinical performance in the Hospice House.
- Promotes safe work practices and work environments to protect the safety and wellbeing of employees and patients.
- Works directly with the Associate Director of Hospice House Operations ensure aspects of daily patient care are effectively managed, including supplies, medications, and contracted services.
- Serves as a member of the broader Partners in Home Care clinical leadership team, and takes part in Agency education, mentorship, and leadership responsibilities as assigned. Serves on the Partners in Home Care QAPI committee.
- Participates in management on call rotation as required.
- Performs other duties as assigned by supervisor.
Minimum Qualifications:
Qualifications include an active RN license in the state of Montana and two years’ experience in a Hospice care setting, including 1 year of experience in a leadership role. Experience in a Hospice Inpatient Unit preferred. Excellent time management, analytical and interpersonal skills required, as well as ability to self-learn, prioritize, and initiate initiatives. CPR certification required.
